Smart watch manufacturers are not in vain trying to focus these multifunctional gadgets on human health. Modern people try to take care of themselves, and smart technology helps them do this not from time to time, but constantly - even around the clock, if necessary. Smart watches are indispensable for hypertensive and hypotensive patients, especially in cases where the pressure can “jump”: on an airplane or while jogging, while walking or hiking. Don't carry a blood pressure monitor with you everywhere.
Such models can also be useful for athletes - they help avoid dangerous overloads during training. A smart watch with a blood pressure monitor is also indispensable for older people: it will help them see a doctor on time.
However, most pressure-measuring “bracelets” synchronized with a smartphone are still not medical equipment. In many models, the tonometer function is just a nice addition. And they, unlike a medical device, do not measure pressure using cuffs, but calculate it like a calculator using a formula, based on the readings of sensors built into the body and biometric data that you enter into a special application on your smartphone. And it works like this: the sensor emits a light signal, it penetrates the skin, and the device records the pulse rate, the rate of dissipation of the pulse wave and the light transmittance of the blood. And then it’s pure mathematics. The more correct the source data, the more accurate the calculations. It was the accuracy of the indicators that we took as a basis when compiling a rating of smart watches with pressure measurement.
How does a watch with a blood pressure monitor measure blood pressure?
The principle of measuring blood pressure with a smart watch is significantly different from measuring it with a conventional tonometer with an inflatable cuff. The watch determines pressure by mathematical analysis of information received from sensors. This is the speed of propagation of the pulse wave, its length, pulse frequency, ECG, etc. After receiving this data, they are transferred to a special mobile application, which automatically analyzes the information received and displays the finished result on the screen of a smartphone or watch.
This method was invented in the middle of the last century by the French scientist Etienne Jules Marey, which is why it was called the “Marey Method”. It is also now called photoplethysmographic blood pressure measurement. But, unfortunately, this method of measurement cannot provide high accuracy and cannot replace the “classical” one.
To obtain more or less accurate values using photoplethysmographic measurements, it is advisable to follow a number of recommendations:
- Before starting the measurement, indicate your physiological data: gender, height, weight and date of birth. This data will allow the software to calculate the data more accurately.
- During the measurement process, try not to move, talk or laugh.
- It is best to take the measurement while sitting, holding your hand at heart level.
- Before measuring, exclude any pacemakers from your diet (tea, coffee, etc.)
- For reliability, repeat the measurement several times.
How does a blood pressure monitor work in fitness bracelets?
The basis of operation of a tonometer in fitness bracelets is slightly different compared to the operation of a medical device. It monitors how the pulse wave dissipates, measures the pulse and processes this information. Next, the indicators are displayed on the screen.
Why can't they be classified as medical devices?
The fact is that ECG technology and indicator technology in activity trackers are different. The sensor inside the bracelet gives results based on the passage of light through the skin at the moment the heart beats. While an ECG gives results obtained during the operation of electrodes located on the surface of the skin.
The tonometer readings are influenced by the following factors: interference from sweat, fat layer of the skin, air humidity.
But it cannot be said that doctors advise giving up fitness bracelets. On the contrary, they encourage the spread of this type of gadget, explaining that the use of such trackers will help a person receive information about his health himself, without having to go to a clinic.
How accurate is the watch's blood pressure readings?
Unfortunately, there are no official statistics on the accuracy of blood pressure determination. According to unofficial data, the average error is within 20% or approximately 10 - 20 mmHg. But, as we have seen from experience, the same device, when measured by different users, can show different errors. Perhaps this is influenced by various physiological characteristics, for example, the depth of the blood vessels, skin color, etc.
Now that we've sorted out the theory, let's move on to the watch itself. In this rating, we tried to collect only the best models of watches with pressure measurement based on reviews on the Internet and our own experience of use. But it is worth clarifying that almost all of these devices do not claim to be a medical device, so the data obtained with their help can be used for informational purposes only.
Popular questions and answers
What parameters should you pay attention to first?
The main thing you need to look at when choosing a smartwatch with blood pressure measurement is the manufacturer, Alexey Sigachev is sure.
— There are professional measuring instruments, and there are toys for fun. If we choose a smartwatch to monitor our own health, it should be more or less specialized. This means that the company producing it must know a lot about medical technology. Before buying a gadget, you can study the company’s website, see what else it produces, and whether it has any idea about medical equipment. Look at the manufacturer, not the brand. In Russia, unfortunately, brands that make semi-professional and professional smart watches with blood pressure measurement are poorly promoted.
It is also worth paying attention to functionality. It is difficult to “fit” many functions into such a small thing as a smartwatch - there is not enough space. But the manufacturer is trying. And then it turns out that the clock works, as they say, at the average level. What accuracy of blood pressure measurement can we talk about then?
Price is also an important factor. Professional measuring devices are not cheap. If the price of a smart watch with blood pressure measurement is low, then we are only dealing with an attempt to bring a budget gadget closer to a medical device. It is also worth looking at the moisture protection index. The most common value is IP67. No matter what the manufacturer writes, they say you can swim in such a watch, I don’t recommend it. These watches are definitely protected from splashes and sweat, but you shouldn’t dive into them. If the index is even lower, draw conclusions.
Last but not least is the error in pressure measurements. If it is indicated on the packaging, the smartwatch is close to medical equipment.
Can you trust the readings of smart watches?
“If smart watch manufacturers do not guarantee the accuracy of measuring results, then no, you shouldn’t trust their measurements much,” says the head of the cardiology department of the hospital. V.V. Vinogradova, general practitioner, cardiologist, candidate of medical sciences Denis Medvedev . — This is explained by the fact that the way pressure is measured on the device is different from a tonometer. Therefore, it is not recommended to use smartwatch readings, say, to adjust a treatment plan or take medications. In my opinion, so far this function of measuring pressure using most hand-held gadgets is experimental. The watch data should be for informational purposes only.
What to do if the smartwatch readings are implausible?
According to commodity expert Alexey Sigachev , it is useless to make claims against the seller or manufacturer. If initially, without even indicating the measuring error of the gadget, the company does not give any guarantees that the readings will be correct, there is no point in waving your hands and swearing, after all, no one promised you anything.
“You need to understand under what conditions the function works,” says Sigachev . – All characteristics must be specified: if this and that is observed, the watch will correctly measure your pressure. And then the user can come and be indignant, saying that I’m doing everything correctly, but “it doesn’t work.” But no one usually prescribes anything like this. The only thing we can advise is not to fasten the smart watch tightly on your hand - the strap can squeeze the blood vessels, and the readings will be incorrect.
In addition, the sensors themselves, if they are of poor quality, can lie - you can try replacing them. You also need to make sure that when measuring pressure they do not move, but are located strictly on the pulse. Plus, don’t forget, each person’s veins are located differently, some have a fat arm, some have a thin one - a certain watch model may not suit one user, but may be the best choice for another. I advise you to just choose more carefully.
Omron HeartGuide
Let's start with Omron HeartGuide, which we already mentioned at the beginning. This is a unique device of its kind, essentially combining a smart watch and a regular blood pressure monitor. During the design period, Omron received more than 50 technology patents related to the miniaturization of all major components (pumps, valves, etc.).
The principle of measuring pressure is practically no different from measuring it with a conventional tonometer with an inflatable cuff, so the results are as accurate as possible. There are air cylinders inside the strap that are inflated during the measurement process. According to the manufacturer, the cuff will withstand up to 30,000 inflations. The device's memory stores the last 100 measurements, and the data is also synchronized with the Omron Connect mobile app.
In addition to measuring blood pressure and heart rate, HeartGuide can count steps taken, distance and calories, monitor sleep and notify about incoming calls and messages. Information is displayed on a very simple and small LCD screen, and the battery life is from 2 to 4 days.
This is where the advantages of this device end. Firstly, for now HeartGuide only works with iOS devices, although there are plans to also support Android devices. Secondly, Omron HeartGuide is not currently sold outside the US - there is no license for it. Thirdly, in the states they want 499 US dollars for them, which is a bit expensive for a hybrid of a fitness bracelet and a blood pressure monitor, although accurate.
Key Features of HeartGuide

ASUS VivoWatch BP
This watch no longer has an inflatable cuff and a system of two medical-grade electrocardiographic sensors and a photoplethysmographic sensor is responsible for the accuracy of blood pressure measurements. To measure pressure, just place your finger on the sensor on the front of the watch for 20 seconds.
Right out of the box, accuracy may not be the highest. But, after calibration, it is within 5%, which is quite acceptable for a non-medical device. In addition to pressure, the watch can measure heart rate, assess sleep quality, track several exercise modes, notify about calls and messages, count steps, calories, and distance. There is an alarm clock and several dial options.
VivoWatch BP is equipped with a 1.26-inch transflective screen and a battery that provides up to 14 days of battery life. They ask for about 200 US dollars for the watch.

Smart watches that can be called the best in terms of battery life. Without recharging, they can last a week, and at full load they can withstand at least two days. The screen is gorgeous: high resolution, large size. The watch can determine the level of oxygen saturation in the blood, and the indicators are similar to those produced by a pulse oximeter. Reviews are satisfied with the brightness and size of the screen, the wide selection of dials, and the ability to answer calls. There is no payment via NFC, and this is disappointing. The model is perfect for both those who are actively involved in sports and those who need a smart companion watch with saturation tracking function.

Samsung Galaxy Watch Active 2
During the launch of the first version of the Galaxy Watch Active smartwatch, there was a lot of talk about the ability to measure blood pressure. But even with the release of the Galaxy Watch Active 2, this feature is still being tested and is available in a small list of countries: Canada, USA, UK, Germany, Australia and Singapore. The measurement is carried out through a special application called My BP Lab, compatible only with the Samsung S9 / S9 + / Note9 / S10 / S10 + line of smartphones.
Otherwise, this is one of the best smartwatches available today. The Galaxy Watch Active 2 is available in two sizes: 44 and 40 mm, as well as in two body finishes: aluminum with a fluoroelastomer strap and steel with a leather strap. This model was the first in the manufacturer’s line to receive a digital touch bezel for more convenient and easy control of the device’s functions.
And there are plenty of functions here. Watch Active 2 can track 39 different training modes, 7 of which are automatically recognized. To monitor activity, the watch is equipped with a large number of sensors: optical heart rate sensor, ECG sensor, accelerometer, barometer, gyroscope, GPS module. The ECG function is not yet active, awaiting approval from the FDA.
The watch went on sale at a price of $300 for the 44mm version (aluminum). The 40mm version costs $280 (aluminum).
In April 2022, the new Health Monitor app was certified by the South Korean Ministry of Food and Drug Safety, and the watch can now be used as a medical tool to measure blood pressure.

GSMIN WP7 watch
GSMIN WP7 is a very presentable-looking watch in a metal case with a laconic design: suitable for both men’s and women’s hands. The silicone strap or metal bracelet is equipped with a high-quality clasp - it will not come undone, and the device will remain on your hand in any situation. The soft material does not rub the skin of the wrist, and you can wear the gadget around the clock. This is very convenient if you use sleep monitoring - the bracelet is very light and does not interfere at all.
The device body is protected from water and dust - IP67 level. You can walk in the rain, and if you drop your device in the water, nothing bad will happen. But you can’t shower or exercise in the pool with him.
The model has a full range of options useful for every healthy lifestyle enthusiast, and a capacious battery ensures uninterrupted operation of the device for up to 7 days. This is very nice, because the watch has a bright and large screen. The screen brightness is enough for good visibility in the sun.
The watch is suitable for supporters of a healthy lifestyle - in addition to standard functions, such as a pedometer and a calorie burn counter, there are additional ones. The device will calculate your pulse rate, measure your blood pressure and even body temperature. Information about your health will help you create an individual training plan that fits your parameters. Smart watches will not allow you to be lazy.

HUAWEI Watch Fit
Advantages: 1. Charges very quickly up to hundred (about an hour) 2. Charge holds for a long time. I charge once a week. I use GPS every day while running. 3. Nice interface, convenient and flexible settings. 4. Auto-detection of training type. Disadvantages: 1. GPS takes a long time to determine. I turn on detection 5 minutes before the “running on the street” workout 2. GPS always determines a different location for the start of the workout, although it is always the same. At the same time, the calculated distance of “running on the street” always differs by a range from 200 to 400 meters on the same route. Comment: I use light. I don’t need an analysis of sleep, stress levels, oxygen in the blood, I don’t need a bunch of additional screens and dials. My requirements are quite meager - correct work during training. For the most part, I am satisfied with this part at 4+.

Samsung Galaxy Watch Active 2
Advantages: In addition to all the advantages, it also shows blood pressure and an ECG with interpretation! Disadvantages: I would like a larger battery and memory. Comment: I had the first model of this watch, it lasted 2 years and something went wrong, the screen began to react every now and then. I started looking for other watches, I even wanted to change the Samsung, but no matter how I looked at the parameters, only Samsung came out, and watch models starting from 50 thousand rubles. I looked at this model for several days, read reviews, looked at reviews and didn’t see much of a difference between the first and second models, only the battery was a little more powerful, the sensors were better, the screen was bigger, and the parameters stated an ECG and blood pressure measurement, but!!️ all the reviews said , that these new functions are in test mode and are not yet active and are already available only in Korea and are just about to receive permission in America.. And how surprised I was when I discovered that these new functions are available. I measured everything, everything was written down in a journal! Only for this you need to download additional. application. Well, in everything else they are also the best and I have not found a replacement for them! I really hope the screen doesn’t fly off this time!

Samsung Galaxy Watch Active 2
Advantages: — Design (subjective); — Functionality; — System shell; — Battery (for my usage scenario); — Synchronization with a smartphone (Samsung C9). Disadvantages: — Original strap; — Fragile (the screen is not protected in any way and it is better to immediately buy some kind of film or protective case for them); — Inaccuracy of the pedometer; — Lack of a pulse oximeter; — Lack of full customization and deep variability of settings. More details about everything in the comments. Comment: The choice fell on this particular watch due to its low price in this segment (I managed to snag it at a promotional price - about $215 for this version). The closest competitor to them at this price is the Huawei watch GT2, which is completely inferior in terms of smoothness of animation and work with a Samsung smartphone (here, after all, the Samsung ecosystem plays a role). Before moving on to further description, I will note that I am using this watch simply as an accessory and as a companion watch, and not as a fitness tracker, so everything related to the sports component will not be described here! Let's start in order: Functionality - it’s easier to name what cannot be done here, namely play media files from messengers; view a specific message, if there are not one, but several, etc. In general, this is not critical, and if your task is simply to be aware that you have received something, then this will do. Also, the watch, although it has such capabilities as controlling the camera, calls and player, recording on a voice recorder, calculator, reminders, etc., lacks such a banal thing as notes. Why is that? After all, the watch has a built-in keyboard and it would be very convenient to write something quickly or dictate, so that the note would later be synchronized to the phone. But no, at best, you have to use a voice recorder instead. System shell - yes, perhaps for some it will not be ideal, but for me it is enough that it does not freeze, does not lag when working with it, FPS does not decrease, as on Chinese analogues, and even though we sacrifice time of use, but Personally, I'm ready to tolerate it. Since we have smoothly approached the battery, a few words about it. With active use with the sensors turned on, the battery lasts for a day or a day and a half. With my use (turned off geolocation, AOD, heart rate measurement and auto-detection of workouts), they last me more than two full days (from 100% to 20%).

HUAWEI Watch Fit
Advantages: - The form factor of the watch, in fact, the device includes the functions of a fitness bracelet without the frills of the older brothers of the GT series, full-fledged smart watches cost completely different money; — They don’t feel big, heavy or foreign on the hand, and don’t create discomfort during sleep; — Large screen diagonal compared to fitness bracelets, which increases the usability of the device; — Sleep tracker and alarm clock; — 12 training courses supplemented with animations demonstrating how to perform exercises; — Music control, although I still haven’t gotten used to the headset buttons and phone volume rockers; — Nice additional functions: flashlight, timer, stopwatch, weather forecast, remote control of the smartphone camera. Disadvantages: - The autonomy of the device with Always On Display disabled, without GPS working, with my not so active use is no more than 5 days; — The track of the built-in GPS is zigzag in comparison with the track that is recorded by the GPS of the smartphone, although this is noticeable only with a large magnification, but it may increase the distance covered; — When driving a car, the pedometer counts steps, which creates considerable inaccuracy in the distance traveled per day. During a 2-hour trip, 8k steps were counted and 5 km of distance covered; — There are few dials that are interesting to me, and only 6 for the Always On Display mode, the dial store operates in the Chinese region; — A small number of characters in notifications, and in rare cases notifications do not appear on the device, although they are in order on the smartphone; — Lack of NFC for contactless payment; — Visible screen flickering at times. Comment: I have long wanted to try this kind of device. Previously, I looked closely at the HUAWEI Band, but was definitely confused by the size of the display. The HUAWEI Watch Fit fitness watch was purchased during pre-order at a price of 5,990 rubles, instead of the official price of 8,490, which is clearly overpriced for this class of device, despite the form factor compared to competitors. I use Watch Fit together with HUAWEI P30 lite, absolutely all the functions declared by the manufacturer work in this combination, which is not surprising. I would like to see new straps for the device appear on sale, expand the functionality of the watch with new firmware, and also eliminate the software shortcomings of a product that has recently entered the market.

HUAWEI Watch GT 2 Sport 46mm
Advantages: They fit comfortably on the hand, I got used to it right away. The thin body does not cling to the sleeves. The “headset” function was very well executed; in fact, it was not needed, but after using it several times, I appreciated how convenient it was, and the microphone worked perfectly. Counts steps well, running and walking are automatically included in training. Accurate heart rate monitor +/-2 beats, or even more accurate. The touch screen is responsive, without lag, and works well even when wet. Slim body, thinner than Samsung. They determine the moments of falling asleep well. Disadvantages: In principle, I haven’t found any as such yet. The battery does not last for the stated 14 days with full use. Emoticons are not displayed in notifications, and there is no way to listen to voice messages. But this is more nitpicking. Comment: They look normal on a thin hand, the battery lasts quite a long time, but not the stated 14 days. All sensors are turned on by default, three training sessions on my hand constantly without listening to music hardly lasted 10 days, but they charge quickly, in less than an hour. They monitor your sleep quite accurately and give recommendations on how to improve it. By default, the watch has a list of 15 types of training + it is possible to add a variety of different ones, after the update the list became so big that I’m too lazy to count. The list of automatically detected workouts is not long: walking, running, cycling, rowing machine. After the update, I again replenished a little, it seems like... Walking and running determine how with the others I don’t know. I would like to add the ability to respond to messages, at least with blanks, and the ability to play voice messages. But you need to understand that this is not a smart watch, but rather an advanced fitness bracelet. But it’s important to me that the watch is on my hand, and not on the charger, so I’m happy with the purchase.

ASUS VivoWatch SP
Advantages: appearance, functionality, screen - especially in sunny weather... the presence of accurate measurement sensors, with an application on the phone is just great for athletes and those who simply care about their health... and also - it is stated that 10 days of battery life without recharging Disadvantages: I did not find , although I tried, but there’s practically nothing to complain about... Comment: there were different smart phones, lg samsung, etc.. in a week you’ll play enough and then you just look at it for a while, which in sunny weather you can’t see a damn thing... with this watch it’s the other way around..... and the accuracy of the sensors is simply amazing... I measured the oxygen level in the blood, it was 93, then I lit a cigarette and measured it dropped to 83 - I need to quit smoking... they monitor it all day and especially at night... very interesting results about the quality of my sleep, without the watch I would never have known so much interesting things about how I sleep.....I can write for a long time, but I think this is one of the best smart watches, nothing superfluous, everything just necessary..IMHO

ASUS VivoWatch SP
Pros: Overall a good watch. They have been working flawlessly for almost a year now, the glass is intact, the strap is intact, that is, the appearance has not been damaged during active use. There are a lot of functions for tracking physical activity. Activities and health status, plus all this is saved in the application, which is very convenient. The only problem with this is that it is not always possible to measure the pressure the first time, you have to repeat it several times. But the indicators are quite accurate - I compared them with a tonometer. Counts the steps clearly and tracks the cycle. After synchronizing with the phone, you can read SMS and reject calls; the function of reading messages from social networks and instant messengers is really missing, plus I would like to answer calls. Disadvantages: Pressure is measured accurately, but sometimes not the first time. Comment: Initially, I was attracted by the design and the stated functionality, despite different opinions in the reviews I decided to take it. As a result, after 2 months there were no problems with them: the Bluetooth connection does not break, all measurements are taken regularly and accurately. With fairly active use, my charge lasts for about 10 days.
